At Valley Youth House, we envision a world in which every young person belongs to a nurturing community. Bridge2Home: a Host Homes model program connects youth (ages 16-20) who are experiencing housing instability with volunteer homes. Valley Youth House developed the Bridge2Home program, in collaboration with the Western PA Continuum of Care Youth Subcommittee, nonprofits, and Offices of Children & Youth to address the needs of the region’s homeless youth.
Using the agency’s expertise in residential services for youth, Valley Youth House provides:
• Recruitment and training to individuals/families that will be Host Homes to youth experiencing homelessness or housing instability
• Background checks for Host Home individuals/families, ensuring the safety of the relationships between Host Home providers and formerly homeless youth
• Case management for youth to help them maintain stable housing after the conclusion of services
• 24-hour on-call crisis support for Host Homes individuals/families
This program serves Pennsylvania youth (ages 16-20) experiencing homelessness or housing instability located in Armstrong, Indiana, Washington, and Westmoreland counties. Youth may be living on the street, in shelters, in out-of-home care, or other situations where youth are unstably or temporarily housed. Additionally, the program may serve youth who are couch-surfing (ie: living transiently with extended family or other adults).
